Please note that these forms are provided in an Adobe Acrobat Reader format that allows you to complete form fields online. Should the form you are completing require signature, you must still print it, sign it and return it in hard copy to the Personnel Office. You may also print the forms for completion offline.

All employees should contact the Personnel Office to obtain the appropriate insurance premium share form to accompany the CIGNA Enrollment/Change form above.

 

 

** Meriden Board of Education employees: All changes are due in the Personnel Office by the 25th of the month to be effective for the 1st of the following month.
